Targeted Case Management at Burlington United Methodist Family Services provides individualized support to help clients identify, access, and coordinate the services they need to live safely and independently.
Serving Garrett County, Maryland, this program helps connect children and adults to critical resources such as mental health treatment, medications, housing, and federal and state benefits.

Targeted Case Management focuses on helping clients:
- Identify personal needs and goals
- Navigate complex systems of care
- Access mental health treatment and medications
- Apply for and manage federal and state benefits
- Secure safe and stable housing
- Coordinate care across providers and services
The goal is to reduce barriers and support long-term stability and well-being.
Program Details
Who Is Eligible
Services are available to children and adults who meet one or more of the following criteria:
- Serious or persistent mental disorder
- Difficulty functioning within the family or community
- Repeat hospitalizations
- Multiple physical and/or mental disabilities
- Homelessness or risk of losing current housing
- Recent incarceration or current parole or probation status
Each referral is reviewed to ensure appropriate and effective support.
