Community Engagement
Accessible, Person-Centered Support in the Community
Services are delivered in the home and throughout the community, making support easy to access and tailored to each individual’s unique needs. Referrals can be completed using a simple one-page form, or a Community Engagement Specialist can assist with completing the referral process.
Who We Serve
Community Engagement Services are available to individuals age 18 and older who may be experiencing:
- Serious mental health illness
- Substance use disorders
- Co-occurring or co-existing disorders
Participants may have a history of—or be at risk for—psychiatric hospitalization, involuntary commitment, or may already be committed and in need of timely, high-quality, community-based services.

The goal is to help individuals remain stable, supported, and connected within their communities while reducing the need for higher levels of care.
